Covered California for Small Business (CCSB) has a fresh approach to offering health insurance to your employees. CCSB is designed with your business needs in mind. CCSB enables employers to remain in control of their health care budget while employees have the freedom to choose a health plan that has the doctors and hospitals that best meet their needs.

Get a quote by downloading the quote template. To complete this worksheet, you will need the name, date of birth, gender and home ZIP code of each employee. Businesses offering dependent coverage will also need this same information for the employees’ dependents. If entering age instead of the birth date, the system calculation may not reflect the true age on the day the group is being quoted.

Complete instructions are included in the quote template. After emailing CCSB the completed template, you will receive an initial quote in approximately two business days.
