You will need Form 1095-A to complete your tax filing. This form shows the financial help (premium tax credit) you received during the year.
When you file your federal taxes (on your own, with a tax professional or using tax software), you will:

IRS Form 8962 and Form 8962 instructions (if filling out your own paper copy)
